Clinical Pharmacy Specialist - Home Based Primary Care

Treats patient, team members, other facility employees, and visitors with respect. Responds to the needs of the service in a courteous and timely manner. Demonstrates appropriate and proactive interpersonal skills to achieve the goals of the service. Acts as a team member and is supportive of the group's efforts at all times. Interfaces with other providers including physicians and mid-level practitioners in designing the Veteran's treatment regimen to optimize efficacy and maintain cost-effectiveness. Serves as an authoritative information source on drugs and their utilization in therapy with special emphasis on medications used for hypertension, diabetes, hyperlipidemia, and anticoagulation. Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions). Reviews the medication regimen of HBPC patients at regular intervals as defined by medical center policy and Pharmacy or HBPC standard operating procedure recommending therapeutic medication regimen changes and monitoring lab tests. Designs a therapeutic regimen or plan, based on patient-specific information that includes a therapeutic endpoint, selecting an appropriate drug, and establishing an appropriate dosage regimen. Provides care that is appropriate to the cognitive, emotional, and chronological maturation needs of the adult and/or geriatric patient. Modifies teaching approach for teaching elderly patients or caregivers when appropriate. Works to develop, upgrade, and disseminate acceptable criteria for use of formulary and nonformulary medications and other high cost items directed at achieving cost effectiveness without compromising the quality of patient care. Reviews medication usage in Home Based Primary Care as part of the overall performance improvement program, and prepares required reports as indicated. Serves as preceptor for pharmacy students and pharmacy residents in accordance with established protocols. Work Schedule:

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Pharmacists must be proficient in spoken and written English. Grade Determinations:
For GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. And must fully demonstrate the following KSA's:
(1) Ability to communicate orally and in writing to persuade and influence clinical and management decisions. (2) Expert understanding of regulatory and qualify standards pertaining to pharmacy. (3) Ability to solve problems coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. (4) Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. (5) Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References:

The work is primarily sedentary. However, some walking, standing, bending, carrying of light items such as books, papers, laptops, etc., or driving an automobile may be required. No special physical demands are required to perform the work.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
